Enrollment Application Request Form 2024-2025 EFWMA School Year

EFWMA is accepting forms for students entering Pre-K 3 through 5th Grade.  Your child must be 3 years of age by August 1st to enroll in Prekindergarten, no exceptions are made to the rule.  
Submit one form per student. 

Forms are accepted from March 1st through June 30th each year. If you complete a form after the deadline, your child will be placed on a wait list that is recorded for your child's grade in August. 

***Please note: All admitted students entering 1st, 2nd, 3rd, 4th, and 5th grade will be administered our local assessment to determine placement for the 2024-2025 school year.  All admitted students will receive a letter notating their date and time for your child to take the assessment.
